Students Asked to Take Part in Survey Regarding Knowledge, Attitudes, and Behaviors involving alcohol use and support of one another

Dear Student,
University of Arkansas researchers are conducting this study to better understand University of Arkansas students’ knowledge, attitudes, and behaviors regarding responsible use of alcohol and support of one another. Sponsored by the National Collegiate Athletic Association (NCAA), researchers at the University of Arkansas would like you to answer questions about images you may have seen on campus, or trainings you may have taken part in, if applicable. Depending on the applicable questions, it is expected that this survey will take between 5 and 30 minutes to complete.
You should be aware that participation is completely voluntary and that even if you agree to participate, you are free to withdraw at any time without penalty. Your relationship with the investigators will not be affected in any way if you refuse to participate. This survey includes questions about you and your knowledge, attitudes, and behaviors regarding alcohol use. Some questions may be uncomfortable to answer, but we ask that you be honest. Your responses are anonymous, which means that they can never be tied back to you as an individual.
Although participation may not benefit you directly, we believe that the information obtained from this study will help us better understand the culture around responsible drinking by University of Arkansas students. This information may be used to develop new programs or modify existing programs to help U of A students make good choices and support each other.
All efforts will be made to keep your personal information confidential and the survey will not collect any identifying information about you (such as your name, birthdate, etc.). We cannot guarantee absolute confidentiality. Any personal information you provide may be disclosed if required by law. Your identity will be held in confidence in reports in which the study may be published and databases in which results may be stored.
Your participation is solicited, although strictly voluntary. If you would like additional information concerning this study before or after it is completed, please feel free to contact us by phone or e-mail. At the conclusion of the study, you will have the right to request feedback about the results.
